Orthodontic services are arranged by The COF Clinic at KIND. The COF Clinic at KIND is a non-profit organization whose mission is to make orthodontic treatment accessible and affordable to low-income families in Colorado by partnering with orthodontic professionals. The participating orthodontic providers of the COF are certified orthodontic specialists who donate their services to children screened and determined to be in need of orthodontic treatment. Potential risks and limitations will be covered by your individual orthodontist, and additional documents will be required by your orthodontist. The COF cannot guarantee results of the orthodontic care provided by your individual orthodontist.
Cooperation: I understand that treatment success depends greatly on the patient’s cooperation. Information and directions provided to the patient, whether written or verbal, must be followed by patients and parents. Lack of patient cooperation can lead to longer treatment time and/or a poor results. Additionally, poor cooperation, bad hygiene, and failed appointments can lead to termination of treatment without refund. Both patients and/or parents need to understand how important it is to follow all instructions given by the doctor and staff.
Appointments: I understand that missed appointments and failing to make routine appointments can result in unwanted tooth movement and /or jaw development which could increase treatment time and may cause you to incur additional fees or termination from the program. It is important that you understand these guidelines and abide by them in order for the best orthodontic care possible.
Oral Hygiene: I understand that proper oral hygiene is critical. Instructions about proper oral care are given at the start of treatment or when indicated during treatment. Lack of excellent oral hygiene may result in decalcification (permanent white marks on the teeth that indicate the early stages of decay), decay, gum disease and bone loss. Regular checkups and cleanings with your general dentist MUST continue throughout treatment. If proper oral hygiene is not maintained during treatment, it may become necessary to terminate treatment before the estimated completion date due to the potential permanent damage to the teeth.
Orthodontic Appliances: I understand that all appliances necessary for the patient’s treatment should be provided by the orthodontist. I understand that if replacement is needed because of breakage, loss or careless handling on the part of the patient, additional charges may be due above and beyond the initial treatment fee.
Retainers: I understand that retainers are worn after treatment to hold the teeth in position. They are worn full-time for 6 months and then at nights after that indefinitely. The initial set is generally included in your treatment, but there may be an additional fee if this set is lost or broken. Please check with your orthodontist.
Discomfort: There may be some discomfort for the first 3-10 days when appliances are first placed. Soft foods may be desirable. Generally, Tylenol or ibuprofen is the recommended pain medication. Check with your doctor.
Additional Treatment: I understand that my child will need to continue his/her oral hygiene appointments with his/her dental provider during orthodontic care. Also, additional specialty appointments may be necessary, such as oral surgery or periodontal surgery. If these services are necessary then you will be referred to a specialist and additional charges may be incurred for these services.
